Programme overview
Introduction:
Geriatric care management breaks down when older clients move between home, day centre, residence and hospital: assessments are repeated or skipped, care plans sit unread, falls and missed medicines go unreported, and families are left to coordinate alone. This Core Concept course equips managers and supervisors of home care, adult day care, assisted living and long-term care services, and hospital discharge teams, to organise assessment, person-centred care planning, safety oversight, family support and service quality. Participants build an Elderly Care Service Improvement Plan for their own service.
Course Objectives:
- Profile the care needs of an older client group using the five Comprehensive Geriatric Assessment domains and the geriatric syndromes that drive demand for support
- Coordinate multidisciplinary assessment and write person-centred care plans with goals, preferences, risks and review dates that frontline carers can follow
- Supervise daily safety routines for falls prevention, nutrition and hydration, medication handling and dementia-friendly care in home and residential settings
- Apply safeguarding, dignity and consent procedures, and escalate concerns about neglect, restraint or financial exploitation of older clients
- Plan assistive technology, home adaptation and caregiver training that keep older clients independent and support families
- Run a home-care or residential service with staffing rosters, quality indicators and incident reviews, and present an Elderly Care Service Improvement Plan
